Comprehensive Health Management
The Case Manager is responsible for coordinating case management and family reunification services for families in an on-site residential facility. Responsible for the safe and timely release of all families to appropriate care. They ensure contractual requirements and client expectations are met at the on-site residential facility. Reports to the Lead Case Manager.
All services shall be provided in accordance with established standards, principles and ethics of the profession, applicable professional specialty organizations, and the high-quality standard for which CHSi is recognized.
This position is for the day shift, 6AM - 6:30PM
Comprehensive Health Services Inc. (CHSi) is assisting in the humanitarian crisis caused by Hurricane Harvey by supporting a disaster relief shelter in Port Arthur, TX. This shelter will house a large number of evacuees who have been displaced by Hurricane Harvey and subsequent flooding.
DUTIES AND RESPONSIBILITIES:
Provides and coordinates case management and family reunification services for children and families
Ensures case managers initiate and maintains direct contact with each family member to efficiently advance the family reunification and release the from the facility
Facilitates the timely release or discharge of family to identified family members or authorized caregiver/custodian and document the provision of services in each family members case file
Ensures assessments are conducted in accordance with FEMA policies and procedures
Reports significant incidents in accordance with FEMA policies and procedures
Works closely with counselors and care coordinators to ensure communication
Maintains the FEMA Database in accordance with FEMA policies and procedures
Other duties as assigned by supervisor
Bachelor’s degree in social work, psychology, sociology, or other relevant behavioral science
3 years of progressive employment experience in the aforementioned fields (advanced degree can be substituted for years of employment experience)
Clean Child Abuse and Neglect (CAN) or child protective services check
Clean criminal background check
Excellent presentation skills as well as oral and written communication skills
Proficiency with computer, common office equipment, and MS Office products
Ability to multi-task, be detail-oriented, be organized and have excellent verbal and communication skills
Immediate, last-minute travel within the U.S. may be required if located outside of shelter location
This is a day shift position with working hours from 6:00am to 6:30pm
Flexibility is required due to potential shift changes based on operational needs and/or command priorities
1 year of experience in family services
Bi-lingual skills are preferred (English/Spanish)
May require bending, stooping and lifting up to 30 lbs
Visual acuity required to complete paperwork and computer work
Able to communicate verbally and listen for constant surveillance of staff activities
Able to withstand changing environmental conditions with weather (rain, lightning, and winds)
Able to withstand and manipulate through construction areas, sports fields, etc.
