Perform comprehensive initial and ongoing assessments of patients/families. Develop initial plan of care in collaboration with 10 teams. Revises the plan of care as needed. Inform the patient upon admission to Hospice of the mechanism for receiving, reviewing and resolving patient complaints. Use assessment skills to determine patients' physical and mental status and status of environment and social support. Identify patient and family health and social issues and potential problem areas. Initiate referrals to other services and community resources as indicated.
Graduate of an accredited school of nursing.
Current Michigan Nursing license.
Two years of recent clinical nursing experience, preferably in Hospice, Home Care, Oncology, Gerontology, or Surgical Nursing.
