SSM Rehabilitation Network is part of Select Medical Corporation's Outpatient Division, a nationally prominent, locally driven provider of outpatient physical rehabilitation with approximately 950 locations in 35 states and the District of Columbia. Throughout the country we operate primarily as Select Physical Therapy, NovaCare Rehabilitation, Kessler Rehabilitation Center and KORT. Our highly respected clinical team provides preventative and rehabilitative services that maximize functionality and promote well-being. We develop individualized treatment plans to help achieve each patient's specific goals. Our integrated local market network allows us to effectively partner with physicians, employers, payors and case managers to achieve optimal patient outcomes in a cost-effective manner. We also provide physical therapy and athletic training services to professional sports teams, colleges, universities and high schools.

Based on the Competency Program established for direct care providers, possesses the skills and competencies necessary to communicate and apply knowledge related to age specific criteria and issues of adolescence, adult and geriatric patients within the context of the job responsibilities and functions. 2. Performs all duties and demonstrates behaviors and attitudes consistent with the Mission Statement and Core Values of the organization. 3. Consistently demonstrates commitment to customer service excellence in all interactions with patients, family members, visitors and other staff that are consistent with performance standards. 4. Continually participates in building and maintaining positive working relationships through effective communication, performance improvement and teamwork efforts. Demonstrates the ability to clearly and effectively communicate (to include reading, writing, and speaking) to all patients, family members, visitors and other staff members with the expectation that each employee, within the context of their specific job duties, can read, interpret, understand, and communicate clearly and effectively in order to ensure proper understanding, all written/verbal communication are followed and information is shared appropriately. 5. Demonstrates, maintains, and upholds expectations set forth in the organizational Code of Conduct. 6. Accomplished individual and team goals and objectives established by supervisor based on prior performance evaluation. Specific Duties and Job Requirements: 7. Evaluates assigned patients and establishes appropriate treatment goals and treatment programs. 8. Provides ongoing evaluation of patient’s status and goals, modifying treatment program for timely progression. 9. Develops comprehensive goals for patient treatment to maximize patient outcomes and return to community activities. 10. Coordinates treatment program in conjunction with other clinical/therapeutic services, as well as available community resources. Attends appropriate clinical meetings to facilitate coordination. 11. Provides for patient education to include development of comprehensive and progressive home programs with instruction to patient, family and other caregivers as appropriate to facilitate a safe and effective discharge. 12. Prepares for patient discharge inclusive of ordering of necessary equipment and recommendations for therapeutic services ensuring the continuum of care. 13. Evaluates patient in actual or simulated home, school, work or community environments as necessary to facilitate return to community activities, making recommendations for necessary adaptations or training. 14. Identifies and manages various levels of care necessary to insure care plan delivery. Delegates and supervises the utilization of COTAs, as well as supportive personnel to maximize patient outcomes and insure effective utilization of services. 15. Carries full patient caseload, ensuring management of patient schedule. Notifies supervisor of scheduled openings and concerns with caseload management. 16. Completes medical record documentation in a clear, concise, accurate and timely manner and ensures accurate and timely charge entry using approved formats in compliance with state, federal and intermediary guidelines and in accordance with established policies and procedures. Ensures care is provided under valid physician orders and within insurance approval, as indicated and necessary for clinical care of the patient. 17. Demonstrates flexibility, team building and active participation in departmental functions in order to promote an efficient and positive work environment. 18. Participates in student program as assigned and appropriate, providing a positive learning environment for students with appropriate supervision, development of learning objectives and completion of student evaluations. Will attend all student lectures, and begin developing public speaking skills required to facilitate student lectures. 19. Identifies and initiates own professional growth, actively pursuing professional development through continuing education, self study and involvement in professional activities and organizations. Participates in a minimum of one continuing education offering relevant to the strategic objectives of the organization and the professional development plan discussed with the manager. 20. Maintains communication with supervisor related to service needs such as necessary supplies, equipment and clinical training in order to effectively complete assigned duties. 21. Actively participates in patient and professional advocacy efforts that may include lobbying activities, health fairs, community level events, professional memberships, fundraising/charitable activities. 22. Adheres to discipline specific State Practice Act and other regulatory agencies. Maintains professional licensure in good standing and completes all state mandated continuing education requirements. Performs in an ethical manner in accordance with discipline specific professional code of conduct.

Bachelor’s Degree in Occupational Therapy required. Current and valid state licensure or eligibility for state licensure as an Occupational Therapist required.
