Job Duties: \r
\tStrictly Telephonic Position\r
\tYou will be responsible for working with\u00a0Special Needs Members and completing the model of care requirements around Health Risk Assessments, High Risk Care Planning\u00a0\r
\tThe target lists typically consist of 300 members each week, there is also a back log of 604 customers that need outreach and documentation on top of the normal weekly volume. You will need to make outreach to the members following our work aides and then create the appropriate care plan\u00a0using note cube, sending letters and documenting in CCMS.\r
\tTime for completion of the entire process from beginning to end can take anywhere from 45 minutes to an hour.\r
\tAnticipated volume would be 8-10 per day. The addition of staff would allow the other CMs to get back in the field and support our membership by providing the CTC Care Transitions\r
\tPerforms prospective, concurrent, and retrospective reviews for inpatient acute care, rehabilitation, referrals, and select outpatient.\r
\tResponsible for educating and motivating employees to participate in wellness programs.\r
\tSupport health promotion and disease prevention and care management services.\r
\tEnsures that case management program objectives are met by evaluating the effectiveness of alternative care services and that cost effective, quality care is maintained.\r
\tManages own caseload and coordinates all assigned cases.\r
\tExcellent communication and interpersonal skills.\r
\tEffective negotiation skills.\r
\tStrong time management and organization skills.\r
\tStrong research and analytical skills.\r
\tKnowledge of managed care preferred.\r
\tRequires two to four years clinical practice experience or certification in case management\u00a0\r
\tNeeds to be a Registered Nurse (RN)\r
