Assists in meeting the psychosocial needs of hospital patients by providing counseling, crisis intervention and complex discharge planning services to patients/families.
KNOWLEDGE, SKILLS AND ABILITIES
Thorough working knowledge of social services in a direct health care setting is needed, including crisis intervention, support counseling and discharge planning
Must possess excellent written and verbal communication skills, particularly in face to face personal contact with patients, families and visitors
Must have knowledge of community information and resources for referrals
Ability to learn simple computer entry and retrieval skills
Exhibits excellent guest relations behavior by telephone and in person
Discretion in handling high confidential information from patient charges and verbal reports, and ability to work with highly diverse patient populations
Must be able to demonstrate understanding of HCA's and St. David's Patients First safety initiative by strict compliance to all safety protocols and procedures
