Clinical Risk Management Senior Advisor
Duties: Implements, reviews and maintains incident reporting system. Routinely identifies risk through review of peer review issues, patient/family grievances, committee minutes, third party reports, attorney requests for medical records and administrative or associate referrals. Performs investigation, including medical records review, and analysis of the frequency, causes and statistical trends of general categories and specific types of adverse incidents causing injury to patients to identify trends, losses and patterns. Reviews all adverse incidents causing injury to patients, visitors or employees as soon as possible and no later than three days. Reviews and analyzes patient grievances for potential trends and risk exposures and makes recommendation for action if needed. Conducts assessment of adverse events, implements appropriate action plans and performs appropriate follow-up of all incidents. Reviews occurrences as required and other pertinent documents, records, etc. to conduct ongoing tracking and trending processes. Formulates assessments and conclusions for the preparation of appropriate reports to be presenting to the appropriate quality committee. Conducts root-cause analysis when indicated on incidents and “near misses.” Develops, maintains and tracks appropriate measures to minimize the risk of injuries and adverse incidents. Works collaboratively with matrix partners to review and resolve incidents that have risk components. Provides counsel and advice on significant Clinical Quality matters; Develops and maintains risk management and patient safety orientation and continuing education materials. Assures new employees receive training within 30 days of employment. Provides annual education and training in risk management and patient safety to appropriate staff. Oversees the implementation of the policies and procedures of the risk management program. Assures submission of incident reporting to appropriate state and/or federal agencies. Serves as risk management consultant with the objective of promoting patient safety and loss prevention activity to protect organizational assets. Serves as a resource regarding accreditation standards including NCQA standards and continuous quality improvement principles as they relate to Risk Management/Patient Safety Complies with policies and procedures and maintains confidentiality of patient information in accordance with state and federal laws.
