Description
The Health Equity & Social Impact Lead (formerly Population Health Lead) will lead and drive Humana's health equity and social impact strategy within the Louisville and Cincinnati markets, as well as serving as liaison to the HESI corporate team. The Health Equity and Social Impact Lead (HESIL) will provide oversight of the community engagement framework for their respective market, tailoring the approach to the needs of the local community based on defined priorities for the market and local community.
Responsibilities
The Health Equity & Social Impact Lead (HESIL) will be responsible for identifying, aligning, and mobilizing the critical internal and external stakeholder relationships that support mutually beneficial needs, goals, and priorities of both community partners and Humana. The HESIL plays a key role in organizing, developing, and implementing annual, quarterly and monthly plans to support Humana's mission and local market priorities by focusing on engagement within targeted communities, growth, access to care, health improvement and overall improved quality of life. The HESIL is responsible for alignment of health equity priorities from local market leadership, including but not limited to, Regional VP Health Services, Market President, Network, STARS/MRA and MarketPoint to ensure goals are being met.
To achieve this, the HESIL must be an outgoing and mission driven professional who thrives on building community relationships, providing resources, and training local associates. They will have experience managing internal and external projects, managing collaborations in a diverse setting, all while leveraging established relationships and knowledge of the key community leaders and Community Based Organizations (CBOs) within the local market, thereby serving Humana's broader membership.
Strong candidates will be curious self-starters who can work in a rapidly changing industry and demonstrate problem-solving skills and cross-functional leadership.
Ability to listen to needs and feedback and develop plans and strategies to work collaboratively to meet the needs of others as well as and Humana
Supports local colleagues in developing and implementing best practices in community engagement and establishing meaningful co-created partnerships
Works with local associates to ensure Humana is visible and has a presence in the neighborhoods of Humana's footprint in the market and works with a variety of organizations serving the community
Executes outcome reporting and monthly reports
Brings the voices representing community interests, needs, and priorities to Humana's local leadership and to HESI to inform program/product planning and implementation
Develops and leads cross-functional leadership boards & aligns strategy with internal business objectives
Maintains active membership in market health collaborative where external organizations have convened workgroups to address local health equity and social needs challenges and community health priorities.
Builds relationships & assesses community needs to tailor the strategy to area health challenges
Works with community stakeholders to connect and build relationships among critical partners in support of community and Humana's business strategies
Gathers and understands multiple community views (departments of public health, providers, universities, social service organizations, large employers, etc.) focusing on a shared purpose, seeking to balance community priorities with business objectives
Manages external partners/vendors and internal matrixed partner relationships
Recommends strategic sponsorship and/or funding expenditures within annual budget and/or identifies and coordinates other internal and external funding resources
Works directly with market leadership, gathering critical information to set health equity goals and priorities specific to the local region
Serves as subject matter expert to internal operations teams to establish and maintain foundational knowledge of health equity and health related social needs (HRSN) into daily business practice
Supports bid planning focused on Health Equity and SDOH related benefits and products

Required Qualifications:
Bachelor's Degree and 5 years of practical experience managing large scale community engagement projects related to public health, health promotion, Medicaid/Medicare, strategic operations consulting roles, retail, supply chain, or other applicable industry or community experience
Proficiency with Word, Excel, and PowerPoint
Strong communication skills
Proven ability to influence and communicate complex ideas effectively, verbally and in writing
Success in developing working relationships within a highly matrixed business environment and in the community at large
Ability to analyze data and make informed recommendations
Competent in project management; ability to manage tight deadlines
Experience developing relationships with key stakeholders to understand and improve the market
Located within the greater Louisville area with required travel to the Cincinnati area as needed
Preferred Qualifications:
Advanced Degree
Experience in Wellness/Well-being/Public Health
Experience in community organizing/non-profit space
Experience in responding to state and/or federal government solicitations
Experience with speaking in front of large groups
