Perioperative Nurse Navigator: Full-Time
Job Summary: The Perioperative Nurse Navigator functions as a member of the multidisciplinary team as an advocate and educator for patients from point of entry, through diagnostic studies, diagnosis and treatment plan for surgery.
The Perioperative Nurse Navigator’s primary function is to build a relationship with patients and physicians, to coordinate a plan of care including appointments, transportation, education, rehabilitation, home care and representation within the multidisciplinary care environment.
The Perioperative Nurse Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each surgical patient including support for interdisciplinary and cross facility collaboration e.g. case management, admission, etc.
On an aggregate level, the Perioperative Nurse Navigator will track performance of the program in line with HCA, division and facility goals (growth, quality, practice guidelines, etc.) and identify opportunities to streamline care practice, for example through development of evidence-based guidelines.
The Perioperative Nurse Navigator will serve as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, referrers, support network members e.g. social workers, and the wider healthcare community. This role will include conducting internal and external outreach and marketing.
During the program development phase, the Perioperative Nurse Navigator will be heavily involved in creation, development and implementation of the surgical services service line programs and the overall surgery programs. It is expected that the Perioperative Nurse Navigator will help identify and expand surgical services offerings and assist with business development
Qualifications are as follows:
o Current State of Utah or Compact Registered Nurse License.
o 3 years of recent perioperative nursing experience
o 1 year management and/or leadership experience
o BSN (4 year program)
o Current BLS certification (available after hire).
o CNOR nursing certification
o 1 year or more of experience as a nurse navigator and/or patient educator in perioperative services
o 1 year or more of experience ins a program development
o Ability to communicate effectively in English, both verbally and in writing.
o Additional languages preferred
o Basic computer knowledge and keyboarding skills required.
o Strong critical thinking skills.
o Current clinical skills.
