Position Summary:

This is an exciting opportunity within the newly launched Pediatric Complex Care Collaborative (PC3):  the collaborative aims to improve the lives of the most complex patients and their families through collaboration and coordination across the care continuum with an emphasis on keeping the patient at home whenever possible. 

In collaboration with the multi-disciplinary team the Case Manager covers the inpatient PC3 population with work focused on aligning care and coordinating care based on the patient/family needs, connecting patient and family with appropriate resources, and works with entire PC3 team to coordinate and streamline patient/family experience.  The case manager will also take part in ensuring and following complex cases across hospital settings and the care continuum.

This position is for an experienced nurse case manager that is innovative, flexible, and team oriented! 


 Pediatric Nursing Case Manager is an expert clinical practitioner and skilled manager of clinical systems that impact patient care:

Coordinates interdisciplinary care for a select caseload of patients from pre-admission through post discharge and taking into consideration the age and development needs of the patient.

Is responsible for the interdisciplinary coordination of patient care as established by critical paths, managed care contracts, and individualized interdisciplinary plans of care.

Mobilizes resources and manages the system(s) in order to respond to concurrent and retrospective variances according to the critical path or interdisciplinary individualized plan of care.

Maintains knowledge of and responds to clinical system, and fiscal data related to specific patient populations. 

Levels I &II. 

In addition to level I, Level II is responsible for:   

Identifies indicators, collects and evaluates data related to case managed population.                                                                                                                             

Leads the development, evaluation, and revision of critical paths, analyzing and utilizing aggregate variances and trends to improve clinical, quality, and fiscal  outcomes.

Preceptorship and onboarding of new Case Managers.

Assists in the development of continuing education materials for case managers.

Provides mentorship and acts as a lead to lower level employees.

Leads committees, as assigned.


Level I:

  Bachelors degree from an accredited school of Nursing                                                                                                                    

Level II: 

Master's degree from an accredited school of Nursing

OR Bachelor's degree from an accredited school of Nursing and at least one required certification stated below.


Level I:

Current registered nurse licensure in the State of Maryland.

Level II:

If candidate does not posses a Master's degree from an accredited school of Nursing, must posses Bachelor's degree and Certified Case Manager (CCM) or Accredited Case Manager (ACM).

Required Work Experience:

Level I:

Three years full-time nursing experience.                                                                                                                                                  

Level II:

Five years full-time nursing experience AND at least one year of full-time Case Management experience.                                                                                                                                              

Levels I & II:

Additional case management/ disease management or managed care experience preferred.

Knowledge, Skills and Abilities:

Utilization management and quality improvement processes.

Demonstrated clinical expertise in acute care nursing with clinical expertise in a specialty area.

Advanced clinical skills in specialty area or equivalent.                          

Levels I & II

Ability to communicate effectively both verbally and in writing.                                                                                                                                    

Advanced interpersonal skills.                                                                                                                                                                                        

Ability to influence and negotiate individual and group decision-making.                                                                                                    

Ability to analyze, synthesize, and summarize information to make judgments regarding patient care.                                                                        

Business acumen so that health benefits and patient/provider satisfaction are maximized while solidifying the program’s viability through cost effective intervention.                                                     

Ability to function effectively in a fluid, dynamic, and rapidly changing environment.
