Sibley Memorial Hospital is a non-profit, full service 318 bed acute care community hospital serving the Washington, DC area. Sibley offers medical, surgical, intensive care, obstetric, oncology, orthopaedic, skilled nursing inpatient services, and a state-of-the-art 24-hour Emergency Department. GENERAL SUMMARY
The Care Coordination RN (Discharge Planner) evaluates timeliness and appropriateness of patient care, and coordinates discharge or transition planning for a smooth progression to post-hospital care.
This position provides coverage every other weekend
ESSENTIAL DUTIES AND RESPONSIBILITIES
Evaluates timeliness and appropriateness of patient care by reviewing medical records, meeting with physicians and other patient care providers, and comparing planned care to industry standard criteria adopted by payers.
Identifies delays in progression of patient care related to tests, procedures, or specialty evaluations through ongoing evaluation of patients and medical records. Negotiates with internal and external providers to promote timely progression of patient plans of care.
Identifies patients with complex discharge planning needs. Recommends options for post-hospital care based on required services and available resources.
Works collaboratively with Case Coordinators, physicians, patients, other patient care providers, and family members as appropriate to facilitate the development of discharge plans. Promotes multidisciplinary meetings as needed to discuss selected complex and challenging cases, and to develop specialized multidisciplinary plans for post-hospital care.
Attends patient multidisciplinary planning meetings with Case Coordinators and other health care team members to facilitate the interdisciplinary care of patients and achievement of positive outcomes.
Promotes the appropriate documentation of case coordination activities on tracking forms, and in the electronic healthcare record.
Educates RN and SW Case Coordinators to accept utilization and discharge planning related telephone orders from physicians, to document online accordingly, and to communicate appropriately to all involved with each patient’s care.
Serves as liaison to payers for providing of information regarding appropriateness of hospitalization.
Serves as liaison to external providers and payers to manage the arrangements for appropriate post-hospital services.
Facilitates transfer of information among healthcare professionals to promote smooth patient transitions between services and levels of care.
EDUCATION AND EXPERIENCE REQUIRED
Licensed to practice as a Registered Nurse in the District of Columbia. Graduate of an Accredited Nursing program.
Bachelor‘s degree required.
Minimum of 2-5 years clinical experience in acute care setting. 3-5 years’ experience conducting inpatient review and case coordination applying industry standard criteria (e.g., InterQual and M&R) preferred.
Current CPR Certification required.
