The case manager utilizes clinical and assessment skills to provide cost effective patient care management. The case manager coordinates services for the patient to ensure timely movement through the health care system. A criteria bases approach is used to determine appropriate use of acute care services. The case manager will identify post acute care needs and refer to the Home Care Coordinator and Discharge Planning/Community Resource Coordinator for discharge planning services.
','!*!MINIMUM QUALIFICATIONS:
Education:
Registered Nurse with a Bachelor's degree, or a highly qualified professional with a Bachelor's degree in the health related field.
Experience:
A minimum of 3 years clinical experience with emphasis in a medical, surgical or ICU care setting.
Demonstrate skill and proficiency in assessing, planning, and managing patient care in clinical specialty.
Equipment/Skills:
Demonstrates proficiency in applying advanced skills in written and verbal communication and negotiations.
Requires positive relationship building skills.
Plans and organizes effectively.
Possesses clinical knowledge of disease process specific to the assigned patient population.
