Performs highly complex (senior-level) case management work in a fast-paced environment to meet the needs of individuals with intellectual and developmental disabilities; Work involves reviewing and tracking case management activities, and assisting in developing program goals, objectives, and procedures; Assigns and/or supervises the work of others; Works under limited supervision, with considerable latitude for the use of initiative and independent judgment. Oversees the development and implementation of clients’ service and treatment plans; Provides technical assistance to other staff members; Reviews clients referred to case management and tracks progress through the case management intake and screening process; Participates in the quality assurance and utilization review processes for case management to ensure quality services; Reviews all case documentation for accuracy and timely submission; Prepares reports on service provider and program activities; Identifies problem areas, service gaps, and areas that are barriers to services for clients; Develops, presents, and conducts presentations and orientations; Documents case records, and prepares and reviews reports and evaluations; Performs contract monitoring activities for the unit; Coordinates the development and implementation of clients’ service and treatment plans; Coordinates transition planning to ensure continuity of care and management of risk factors; Assists in the development of policies and procedures; Assists with program monitoring and evaluation, including production targets and performance metrics; Assists with intra-agency or interagency services coordination and referral problems; Assigns and/or supervises the work of others; Conducts performance reviews; Make recommendations regarding hiring, promotion, and termination decisions; Disseminates information to the team and forwards information to the manager in a timely manner; Works closely with team members to resolve customer problems; Develops the team to ensure their consistent delivery of required performance measures.
Desired Degree/Job Experience:
Two (2) to three (3) years’ experience in case management or Job Experience services work; Graduation from an accredited four-year college or university with major coursework in social, behavioral, human services, or a related field is required, as defined in Texas Administrative Code, Title 40, and Rule 2.559 (b) (1); Prefer ability to meet specific requirements of Title 42, CFR, Section 483.430; Prefer Two (2) to three (3) years’ experience working with Individuals with intellectual and developmental disabilities; Prefer bilingual fluency in both English and Spanish.. This position is a Pay Grade 12, with a starting salary of $40,555.55 annually. This is an exempt position. Open until Filled.
