Responsibilities

Coordinate and identify care needs (gaps in care) for patients attributed to a primary care practice.

Identify patients through the health plans gaps in care list, state immunization databases, the patient medical record and the use of a patient disease management registry.

Collaborate with the providers and review care needs and validate these needs against the medical record.

Reach out to patients with validated gaps in care and facilitate meeting these needs.

Support quality outcomes and high reliability in the Patient Centered Medical Home.

Requirements

Licensure / Certification / Registration:

BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.

Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Additional Preferences

Candidates with patient outreach experience and strong computer skills are preferred.
