Description
The Senior Program Delivery Professional will lead the IHWA new initiatives with our partners for driving improved member and provider experience as well as improving overall assessment completion rates.
Responsibilities
The IHWA Senior Program Delivery Professional will be responsible for driving the new initiatives as identified and approved with our partners and internal stakeholders.
The In-home Health and Well-being Assessment (IHWA) Program works with internal partners (YHA) and external partners to complete assessments with members in their homes. This visit with the member brings several types of value to the member and to Humana, including:
Care management referrals to clinical programs and other member resources
Medication review and issue resolution support
Clinical quality screenings
Member and home environment observations, such as tripping hazards and SDOH concerns
Summaries provided to the member and their physician with gaps identified for follow-up
Assessment and identification of the member's chronic conditions
Key responsibilities include:
Project Management of new initiatives
Ability to set clear expectations with accountable parties, including securing commitments to support new initiatives
Collaborate with Humana leaders, vendors, and the IHWA team to review and prioritize additional short and long term solutions to deliver new initiatives
Partner with IHWA Data Integration SME to define IHWA data use cases and support delivery as appropriate
Managing and prioritizing the pipeline of new initiative requests
Developing high quality analysis clearly framing new initiatives objectives, issues/challenges, and articulate compelling, insightful findings, conclusions, and recommendations
Continually assess opportunities for piloting test and learns and expanding existing technology/data for driving ongoing IHWA success

Required Qualifications
Bachelor's degree or equivalent work experience; ideally in healthcare or clinical operations
2 years of project management or project implementation experience
Strong leadership and organizational awareness with ability to facilitate and resolve conflicts
Strong presentation and interpersonal skills
Comprehensive working knowledge of all Microsoft Office applications, including Word, Excel, Access, PowerPoint, Visio
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Preferred Qualifications
Master's Degree
Experience with Azure or RMS
PMP or Six Sigma certified
Understanding of Humana clinical programs and models of care
