POSITION SUMMARY:Responsible for the facilitation and coordination of hospital activities related to concurrent review, including length of stay certifications, monitoring and response to issues and trends affecting reimbursement; monitoring and data collection for physician utilization, quality and peer review; the facilitation and prioritization of patient discharge planning needs, including equipment and supportive services, alternate care facility placement, resource analysis and planning.

POSITION QUALIFICATIONS:

EDUCATION:Required: Bachelor Degree in Nursing, Social or Health Sciences, Business or Healthcare Administration or related field.

EXPERIENCE:

Required: Minimum six years of clinical experience within acute care setting, encompassing both direct care and managerial responsibilities. Knowledge of utilization review process, case management philosophy, process and models, principles and terminology of managed care.

KNOWLEDGE, SKILLS AND ABILITIES://Ability to communicate, mediate and negotiate complex issues with a high level of organizational skill. Respects and maintains confidentiality of patient information obtained relative to case management activities. Can work independently with minimal direction, seeking consultation and guidance as appropriate. Functions as case management liaisons with customers of the health care system, both external and internal. Operates office equipment including but not limited to: telephone, computer keyboard with access to management information systems and tools, order communications printer and fax machine. Effectively communicates via written, verbal, face-to-face, telephone and computer methods.Recognizes that variables such as age, race, nationality, culture, disease-causing vulnerabilities, socioeconomic factors affect the patient populations under their care as defined in the unit’s provision of care statements. Uses interventions appropriate to these factors (e.g., interpreter, dietary preferences, social service consult, etc.)
