Community Care Alliance is activity seeking qualified candidates for full-time Outpatient Enhancement Case Manage position in their Family Behavioral Health Department.

The Outpatient Enhancement Case Manager will provide individual and family services to children's and adults in out-patient level of care;  services will take place in a variety of settings.  The outpatient Enhancement Case Manager functions as a part of a team and are required to collaborate with other team members and individuals within our agency as well as with outside agencies and individuals involved with families.

Education/Licensing Requirements:

  Minimum Associate degree in Human Services, Bachelor's in Social Work or related field preferred; Driver's license.
