SeniorBridge Home Care is looking for social workers who want the flexibility of Per Diem work and the reward of helping Seniors and their loved ones. Case work available in NY City and Queens.
The Home Care LMSW assesses and evaluates client's needs and requirements to achieve and/or maintain optimal wellness. They guide clients and their families toward and facilitate interaction with resources appropriate for their care and wellbeing.
Responsibilities
The Home Care LMSW employs a variety of strategies, approaches and techniques to manage a client's physical, environmental and psycho-social health issues. The Home LMSW work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. This is a rewarding and impactful role, and responsibilities include the following:
Identifies and resolves barriers that hinder effective care.
Ensures client is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
May create client care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
Follows established guidelines/procedures.
Required Qualifications
Licensed Master of Social Work in the state of New York
1 years of home care or related healthcare experience
Must have a strong understanding of long term care and financial planning
Good communication skills both written and verbal
Knowledge of community resources
Ability to work in a multi-disciplinary team
Proficient with Microsoft applications: Outlook, Word and Excel
This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Preferred Qualifications
Experience working with the adult and geriatric population
Experience working with clients who have dementia
Enthusiastic, positive thinker with effective organization and communication skills
