Serves in a dual capacity: 1) Providing comprehensive
chemical dependency related services (including assessment, education, brief
treatment, outpatient counseling, case management services, and related
therapeutic services) to adults residing in shelters, transitional/permanent
housing facilities, and in locations not fit for human habitation. Participates
in outreach services to specific targeted populations, included but not limited
to, Spanish-speaking individuals and refugees.



 

ESSENTIAL FUNCTIONS:

·        
Completes an initial assessment that includes an evaluation that
utilizes the American Society of Addiction Medicine Patient Placement Criteria
(ASAM) to identify
patterns of substance use disorders.

·        
Performs a comprehensive assessment
(bio-psychosocial history) to analyze and synthesize the client’s status and
addresses the client’s strengths, problems, and areas of clinical concern. On
an ongoing basis assesses client needs, strengths, behaviors, and progress
toward desired outcomes.

·        
Monitors client progress in areas of personal
growth, education, employment, living skills, etc. Determines the need for
outside professional evaluations (e.g., mental health), and independently
refers to community practitioners/agencies as needed.

·        
Conducts educational groups
providing recovery related information and instruction.

·        
Provides crisis intervention/management services
and conflict resolution assistance.

·        
Provides
counseling/therapy services in individual and group sessions.

·    Serves as a case manager for assigned
clients.

·    Collaborates with all providers to ensure
holistic services are provided. Collaborates with all involved individuals in the
provision of care to ensure continuity and maximum effectiveness of care/services
provided.

·    Participates in the development of treatment plans in
accordance with program requirements, state regulations, etc. and communicates
pertinent treatment plan information to other staff as needed. Updates and
revises client treatment goals/objectives on a regular basis consistent with
House of Mercy and regulatory expectations.

·    Participates in regularly scheduled care meetings to discuss/update
staff on client progress, status, and treatment needs. Professionally
communicates with other program staff to promote team collaboration, input, and
involvement in problem solving/resolution insuring consistency in the implementation
of treatment plan goals/objectives.  

·    Effectively communicates client service needs with community
agencies, schools, courts, service providers of significant others, and mental
health/other health care professionals to provide an extended, high quality
continuum of care.  

·    Collaborates with treatment team in
developing appropriate discharge plan and aftercare programming. Assists clients
in their efforts to locate and obtain appropriate housing post House of Mercy.  Integrates the resident into appropriate post-placement
programs including Alumni and Continuing Care groups.  

·    Prepares and distributes required reports and other documentation
of services according to House of Mercy policies and procedures, applicable
laws, regulations and licensing requirements.

·        
Responsible for the maintenance of
client files. Assures all needed releases of information and other paperwork
are present in the file and up-to-date. Promptly requests new or additional
information and paperwork as needed.

·    Responds to volatile situations by mediating to resolve issues
and to model appropriate crisis intervention and problem solving techniques.

 

MARGINAL FUNCTIONS:

·    Provides statistical and research information as requested and/or
required by laws, regulations or licensing requirements.

·        
Participates in House of Mercy’s Performance Quality Improvement Process.

·    Participates in public relations and media efforts to promote
House of Mercy; attends community and informational meetings to keep House of
Mercy current and to network with other agencies; attends training on relevant
topics to stay current in meeting requirements of licensing and other
regulations, and to obtain information to better serve clientele.

·        
Participates in the education and training of other
program staff, interns, residents, other students and providers in coordination
with supervisor.

·        
Provides coverage as
assigned.

·        
May drive locally.

·    Assists the Director in the development of programs and/or
curriculum, as directed.','!*!MINIMUM KNOWLEDGE, SKILLS AND ABILITIES
REQUIRED:

·    Bachelor’s degree in a human services related discipline from an
accredited four-year program, required.

·        
CADC Certification or
obtained within twelve (12) months of hire.

·        
Valid Driver’s/Chauffeurs License required, must
meet Mercy’s Motor Vehicle Safety Standards, must be at least 18 years of age
and be eligible to drive per Iowa state law.

·        
Proof of
completion of Mandatory Reporter abuse training specific to population served within
thirty (30) days of hire.

·        
Ability to understand medical
