Job Description Summary
The Manager, Care Management leads teams of social work and social service professionals responsible for contributing to the multi-disciplinary care management team. The Manager, Care Management works within specific guidelines and procedures; applies advanced technical knowledge to solve moderately complex problems; receives assignments in the form of objectives and determines approach, resources, schedules and goals.
Responsibilities
The Manager, Care Management oversees the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area. Requires cross departmental collaboration, and conducts briefings and area meetings; maintains frequent contact with other managers across the department.
This position requires an experienced individual with strong management or supervisory skills, as well as a health care background, with the authority to ensure that the staff meets all productivity, quality and timeliness standards. The Manager must be able to lead change and promote a positive team attitude and be able to effectively respond to unexpected changes or demands. Must be passionate about contributing to an organization focused on continuously improving member experiences. Must be able to work independently and make sound judgments within the scope of work assigned, understands when to seek assistance from professionals and management, as required and appropriate outside of the scope of work and/or within the multi-disciplinary team providing services to beneficiaries.
Required Qualifications
Master Degree in Social Work
Active Social Work (SW) license
5 years of professional experience in care management, geriatric care or related experience
2 years of people management experience
OR
equivalent experience. Equivalent experience includes being a Team Lead, SME, Team Champion, or mentor
Proficiency in analyzing and interpreting data trends.
Progressive business consulting and/or operational leadership experience
Comprehensive knowledge of all Microsoft Office applications, including Word, Excel and PowerPoint
Must have a separate room with a locked door that can be used as a home office to ensure you have absolute and continuous privacy while you work
Must provide a high speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 25x10 (25mbs download x 10mbs upload) is required
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Ability to travel, work and train onsite plus additional travel as needed for ongoing training when permitted
Preferred Qualifications
Previous experience working in a managed care field.
Experience in budget planning, tracking performance against daily and weekly metrics, and operational experience

For this job, associates are required to be fully COVID vaccinated (preferred) or undergo weekly COVID testing and wear a face covering while at work. Interview Format

If you are selected to move forward from your application prescreen, you will receive correspondence inviting you to participate in a pre-recorded Voice Interview and/or an SMS Text Messaging interview.  If participating in a pre-recorded interview, you will respond to a set of interview questions via your phone. You should anticipate this interview to take approximately 10-15 minutes.
If participating in a SMS Text interview, you will be asked a series of questions to which you will be using your cell phone or computer to answer the questions provided.  Expect this type of interview to last anywhere from 5-10 minutes. Your recorded interview(s) via text and/or pre-recorded voice will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.
