POSITION SUMMARY:
· Provide support to children and youth who are at risk of hospitalization to develop the necessary skills, coping strategies needed to become independent and lead fulfilling lives.
· Services take place in a variety of setting and are intensive in nature
· Psych rehab workers function as part of a multi-agency team and are required to collaborate with other team members as well as other agencies, community members and individuals involved with families.
EDUCATION AND TRAINING REQUIRED:
· Bachelor’s degree in a Human Services field preferred
· Driver’s license required
· Bi-lingual candidates preferred
· If applicable, employee will assume full responsibilities for the cost and efforts of maintaining all forms of licensure, certification, and credentialing documentation as required in their job description or sited on any action form as a qualification for hiring, job promotion, or monetary increase. Subsequently, the employee will adhere to the credentialing requirements of all insurance/funding sources for which they qualify and provide Human resources with the initial required documentation and all renewals of these documents thereafter.
WORKING CONDITIONS:
· Work takes place primarily in client homes or other community-based settings, and occasionally in an office setting.
· Ability to communicate effectively in person and in writing
· Must be able to flex working hours to meet client needs
