GENERAL OVERVIEW:
The primary responsibility of the Access Nurse is to coordinate the customer oriented inpatient access for outside hospital transfers. In collaboration with Access Services; he/she also helps facilitate timely and appropriate assignments of patients to all the Cardiovascular Center inpatient beds and from all admitting sources. These admitting sources are outpatient clinics, outside facilities, operating rooms, PACU and procedural areas.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Acts as primary liaison for referring physicians and BWH MDs, and is responsible for decision-making about timing and urgency of admissions for patients.
Coordinates hospital to hospital transfers and outside facility ED to BWH transfers in collaboration with the ED flow manager.
Elicits patient information from referring MD and outside facility nurse to allow appropriate patient triage and placement.
Establishes and sustains successful external relationships with ancillary departments and referring physicians to assure smooth and efficient functioning of the schedule and excellent customer service to our referral base.
Notifies team and inpatient service of admission via email to attending physicians; expect note placed for house staff.
Facilitates referring physician callback for specific information.
Provides general information about the program and expertise available for both the medical and surgical services.
Receives back-up and decision support from the ED attending physician for potential inpatient admissions.
Interfaces with patient access services to provide through-put and bed utilization services to enhance the patient experience, while optimizing patient flow.
Reviews bed availability frequently, assigns patients, in collaboration with patient access services to appropriate beds based upon bed availability and triage information.
Uses information to make proactive patient placement decisions
Updates physicians and transferring facilities on bed availability
In conjunction with Patient Access Services, maintains QA of patient access information.
Serves as a representative of organization and consults with appropriate administrators, patient care unit leadership, procedural unit leadership and access services during extreme census.
Collaborates with Off-Shift Nurse Administrators regarding ED, procedure and inpatient areas
Completes written communication from OSH to accepting BWH RN to initiate and streamline the transfer process
Maintains/completes transfer booking sheets to maintain the tracking database
Qualifications
QUALIFICATIONS:
Education: Graduate of an approved school of nursing BSN required, MSN preferred. Current license to practice as a registered nurse in the State of Massachusetts.
Experience: 5 years adult critical care and or emergency department experience preferred. Must have extensive knowledge of patient flow and outside hospital triage guidelines. Nurse leader experience required such as: flow coordinator, access nurse, bed control nurse.
Continually updates clinical knowledge and skills through formal and informal education and review of the current literature.
Must work well independently, and have sound decision making skills
Must exercise a high degree of professional judgment within the scope of licensure
Possess interpersonal skills to interact effectively with intra-inter hospital physicians and personnel
Possess ability to manage critical issues relating to patient care and changing organizational climate
