Senior Director, Quality and Safety
Duties: Aggregates and communicates the quality, risk and safety trends to the Senior Clinical Leadership team on a monthly basis. Tracks and trends the safety data, and develops appropriate action plans and risk-modification strategies. Responsible for the investigation, management, and reporting of clinical events consistent with the Kindred’s Policies. Respond to significant occurrences; review facts; review medical record/documentation; preserve any evidence; interview staff, patients and family as indicated. Oversees the work of the Quality and Safety directors to equally distribute the workload, monitor the quality of the output and ensure seamless teamwork within the department and within each division. Coordinates and develops along with the clinical leadership team the education and training based on the quality and safety trends in the division. Assists in the development and implementation of patient safety and performance improvement programs. Helps to oversee the investigation findings quality and safety team. Serves as a safety/performance improvement resource by collaborating with personnel and departments involved in patient safety and performance improvement activities. Review incidents and determine whether constitutes Serious Preventable Event under the Patient Safety Act and or Safe Medical Device Act. Develops strategy for execution of reduction of professional liability losses. Assist with the completion of the annual risk assessment, mitigation and strategic plans for performance improvement in quality, improving patient satisfaction and promoting a zero harm culture. . Demonstrates and applies knowledge of performance improvement processes and regulatory standards to provide project support and data collection assistance for the improvement of organizational or departmental systems and processes. Monitors the quality of the documentation on investigations and in Kindred’s risk management software to improve the accuracy of data collected, the conclusions drawn and root-cause analysis. Supervises the reporting of potential claims to liability claims and the division’s Chief Counsel. Maintains logs of educational and intervention activity and reports monthly through confidential data base.
