This group is powered by Physicians, and an engaged support staff focused on quality patient care. Nurse Practitioner or Physician Assistant experience required. Experience in a post acute setting or geriatrics preferred. 

This Opportunity Offers:

• Full time position, Monday through Friday, every fourth weekend
• Seeing nursing home patients in post acute setting 
• System-wide Epic EMR
